Registered Dietitian

Searching for a self motivated Registered Dietitian to provide consulting services to long term care facilities. Responsibilities include completing nutritional assessments, quarterly kitchen audits, assisting with in-servicing of staff, attending clinical meetings as needed.

Required education:

  • Bachelor's

Required experience:

  • Clinical: 1 year

Required license or certification:

  • RD,LD/N

Licensed Physical Therapist

We are looking for a experienced therapists that aligns with our mission to maintain health and retain full functionality. Must have strong proactive skills and excellent treatment interventions, especially with cardiopulmonary and medically complex patient population. Uses evidenced-based assessment and treatment tools.--Collaborating with homemanager and entire home health team on treatment and care planning
